I work for a Costa retailer and they offer the lense replacement at wholesale cost to you, so it will depend on which lenses you have but usually $10-$20 covers it.

I wear patagonia rock craft pants, a magellan long sleeve shirt, a sun runner hat from Outdoor Research and a UV buff. If I'm gonna be out all afternoon I'll wear a pair of gloves that Buff makes. Hope this helps!
