Hook set problems

So i'm new to bass fishing and I have only ever caught bass on the senko worms on a wacky rig. However, everytime I set the hook, it ends up coming out around the nostrils. How should I change my hook setting technique. Also today I caught a bass and I broke its mouth do you think it will live?

    Were you holding it by it's jaw?  Not a good practice if you're not supporting most of it's weight by it's belly as well.

    Yeah but it was less than half a pound and it came up with the broken mouth
