for the 1 fish issue use a O ring and then use a circle hook this will help solve... 1 the o ring will let you get multiple fish per bait as the bait is not being pierced at all.. 2.. the circle hook will most times get hooked in the corner of the basss' mouth eliminating the gut hooking.. If you have To T rig the bait watch your line a lot more....... seems like you are not carefully doing this and the fish is getting gut hooked..
