Just happened to come across it reading an article a few minutes ago. I thought "hey...that looks like the worm that needed some ID on the forums" lol.

I think they may at one time had been sold as "zetabait ding-a-lings" but I guess that mold is then only way to get some now and I'm not about to start pouring my own plastics haha

Make your own mold of it, plaster of paris and a disposable storage container. 

Start contacting different plastics pourers and see if the can get the mold. 

Your worm is a production paddle tail not hand poured, the nose has a sprue.

There are two piece mold available for pouring as well that will have a sprue. I pour stick baits with a two piece plaster mold that I have to cut the tip off of after pouring. The mold he linked to its an injection mold as well.

I remember years ago a company called Producto that made worms like these.

Couldn't tell ya what worm that is,but there are many variations of that worm if you want to try another... But you can make a mould of that worm and create your own replicas.
