I was given this bait by a local bass fisherman.....but I have no idea what it is or who makes it

no it's not.......it didn't come on a hook.

Could be a banjo minnow. They made some with paddle tails.

Almost looks like a Big Bite Baits swim minnow, but the bottom fin is different

I look at the saltwater catalogs each year for crossover baits and different colors offered in some cranks. 

Its a saltwater bait, the plastic is alot harder right?

Now Im going to answer the question you were going to ask: will it work on bass ? the answer is yes.
