Help with identifying Hardbait

Hi I've been trying to find the brand to acquire more of what I believe was a jerk bait. Father in law gave it to me and I caught alot of bass and a few Pike on it before my daughter cast it into a tree !!!, have spent hours sifting through brands with no luck. Pic attached. Lure would gave been purchased 5 plus years ago. Thank you

I have found in most cases that front hook rarely ends up inside there mouth and hooks em in the head and stuff as they fight. I usually remove the front treble when they have 3

Good to know. It had already been removed on this one. I'll admit that was the first one I've used and haven't Fished them alot but on a shadow rap deep I had multiple times hooked on trailing hook in the mouth and the lead hook dug into its head where the d**n trebles almost worked against each other as far as removing one dug tgenother in further. Perhaps I will look into this more as I took one person's suggestion.  Thank you
