Floating or slow rising shallow 4 1/2" to 6" (or Royal with cheese equivalent) with only 2 hooks. Seems like most have 3 hooks. I'm looking for ideas, not necessarily to buy one here, although...

a slow floating shad rap should work. 

 

I have used and liked the yozuri bait mentioned above too. 

 

Shad rap is fatter/biggest, duo is smallest but casts like a bullet for its size, yozuri is most reflective, bigger than the duo, more slender than the shad rap, slimmer profile than the shad rap. 

X-Rap is the largest jerk bait I have with only two trebles, at 4.75", and Yo-Zuri Crystal Minnows just slightly shorter.

 

 

Can third the Shadow Rap Shad. It's a bit shorter than you specified but it gets bit.

If you can find a Lucky Craft Spazz, they are an excellent very shallow running jerkbait with 2 hooks. I have a couple I use for trout in shallow rivers and streams. The 3 3/4" bait is listed as a 1-2 foot diver, so very shallow running. 

I think 3  trebles is overkill anyhow. I take the middle treble off on all mine and it does not effect the action. They still catch just as many fish. I have never missed a fish with the middle hook removed. It is easier to release the fish with less damage to him and less chance of a hook in you. Try it, you can always replace it later if you want.
